- This Voluntary Vision plan is through UniView Vision (an Anthem Company), one of the largest vision care networks in the industry. When you choose to receive care from a UniView Vision participating provider, you receive full in-network benefits and discounts.
- Coverage is for routine eye care only; if you need medical treatment for your eyes, you must visit a participating eye care physician from your medical network.
- UniView Vision providers cover a wide range of ophthalmologists, optometrists, and opticians, including retail locations such as 1-800-CONTACTS, LensCrafters, Target Optical, Sears Optical, JCPenney Optical, and Pearle Vision.
