Voluntary Life Insurance

This benefit is administered by CIGNA.  New employees may purchase in increments of $10,000, two-times their base salary or $200,000 of life insurance coverage (whichever is less) with no medical questions asked (guaranteed issue).  New employees are eligible to request additional coverage over the guaranteed issue in $10,000 increments up to five-times their base salary or $500,000, whichever is less, after completing the evidence of insurability process and being approved for the coverage.   The evidence of insurability process requires completion of a medical questionnaire.  Based on the information provided in the questionnaire CIGNA may request additional tests or copies of medical records to determine eligibility for coverage.  Employees will be notified in writing by CIGNA of any additional requirements as well as the approval or denial of their coverage.

 

New employees may also purchase life insurance for their spouse/same sex domestic partner in increments of $10,000 with no medical questions asked, up to $30,000, or up to a total of $50,000 with completion of evidence of insurability.    

 

Employees may purchase life insurance for their dependent children in increments of $2,000 with no medical questions asked, up to $10,000.

Cost

Paid entirely by the Employee.  The cost of coverage is based on the applicant's age and the amount of insurance desired. 


Enrollment

A full-time Employee is eligible to enroll on their first day of employment.  However, coverage does not begin until Human Resource Services (HRS) is in receipt of the completed application for enrollment.   The guaranteed issue amount goes into effect as soon as HRS receives the application.  Any additional coverage above the guaranteed issue amount is not effective until approved by CIGNA.

 

 A part-time Employee who becomes full-time is eligible on the first day of full time employment.


In order to enroll in this plan an employee must complete and return to HRS a completed application no later than thirty-one (31) days after his/her eligibility occurs.  Failure to comply with the application requirement negates the Employee's eligibility to participate until an open enrollment period takes place. 

 

All requests to add coverage after initial eligibility will be subject to medical underwriting for any and all amounts.  This applies to new hires who declined participation at initial enrollment as well as increases in coverage to those currently participating in the insurance.  These requests can only be made during Open Enrollment which occurs each year prior to January 1.   Such changes, if made, will be effective upon approval by CIGNA for coverage requested.  Unless a qualifying condition is met, enrollment in this plan cannot be changed during the calendar year in which the election is made.
